Residential Medication Management Reviews (RMMRs)

RMMRs are medication reviews conducted for patients who reside permanently in aged care homes (previously called residential aged care facilities (RACF)). RMMRs can be either collaborative (initiated by the patient's (usual) GP and carried out by an accredited pharmacist) or routine (initiated by an accredited pharmacist as part of a contract with the facility).

RMMRs are not available to in-patients of a hospital, a day hospital facility, people receiving respite care in a RACF, or people living in the community setting. A HMR is the appropriate review process for such categories.
